Healing Path Recuperative Care is a holistic, patient-centered recovery facility that provides short-term housing,
medical support, and social services to individuals recovering from illness, injury, or medical treatment. Our policies
and procedures ensure the highest quality of care guided by National Institute for Medical Respite Care (NIMRC)
Mission Statement
To provide a nurturing and structured recuperative care environment where individuals can recover Post acute hospitalization.
Admission Criteria
To qualify for admission, individuals must:
- Be medically and psychiatrically stable.
- Require temporary recuperative care and support.
- Independently manage daily living activities or have minor assistance needs.
- Self-administer medications.
Be cleared from any infectious disease transmission-based precautions. - Be committed to engaging in the recovery and housing transition process.
Exclusion Criteria:
- Active substance use requiring detox services.
- High-level medical or psychiatric needs that require hospitalization.
- Individuals presenting a danger to themselves or others.
Service Model & Care Coordination
Healing Path operates on a multi-disciplinary model, incorporating:
Medical Case Management
- Initial nursing assessment (vital signs, wound care, medication review).
- Ongoing medical monitoring and adherence to treatment plans.
- Coordination with primary care providers and specialists.
Social Services Coordination
- Comprehensive behavioral health assessment.
- Individualized recovery and housing stabilization plans.
- Connection to community- based mental health and housing programs.
Housing Case Management
- Early assessment of long-term housing options.
- Assistance with document readiness for housing
- Support with benefit applications and eligibility verification.
Psychosocial & Emotional Support
- Individual and group counseling with licensed therapists.
- Support groups for stress management and coping skills.
- Referral to peer recovery support services.
Daily Living Assistance
- Semi-private suites with modern amenities.
- Daily nutritious meals prepared based on dietary needs.
- Support for activities of daily living (ADLs) as needed.
